Fees and Forms
In order to spend more time on “client-focused” care, I do not participate in healthcare, managed care or
HMO insurance programs. However, I will provide you with a super-bill that contains the information
necessary to submit to your insurance company if you like.
The value of private-pay services over managed-
care services are that they are more
personalized, removing the stigma of the medical
model and clinic setting, offer true privacy and are
more effective.
Payment is due on the date of service by check or cash. Under the rare case of a check bouncing future
payments will only be accepted by cash. Phone payments will be arranged individually.
Cancellations require 24 hour notice.
